Oral Surgery Consultation
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Patients desiring I.V. Sedation or General Anesthesia require a consultation visit prior to the day of surgery.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- New Patient Registration
- A list of medications you are presently taking.
- Knowledge of medical conditions, allergies, and past procedures requiring anesthesia.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian at the consultation visit. This is required by state law.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valve, artificial joint, heart disease such as rheumatic fever, heart attack, bleeding problems, immune system deficiency, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. X-rays must be recent enough and of sufficient quality to be diagnostic and satisfy the “standard of care” for the planned procedure. For removal of impacted wisdom teeth a panoramic x-ray dated within six months is required. If additional films are necessary, they can be taken at our facility.
Referrals
If you have been referred to our office by your dentist or doctor, please bring your referral slip with you. Referral documentation helps to ensure continuity of care between health care providers. Having this information available will shorten your appointment and the planning process for your care. Depending on your situation, a referral may not be needed, or it may be required in order to complete treatment. If you do not have a referring doctor, but need one, we can advise or sometimes arrange this for you.
